Partner questionnaire links
Questionnaires submitted through a funeral-home link are saved for Dana Productions and that funeral home’s authorized staff. Order and billing records are kept in the private partner portal. Only Dana Productions and approved staff at your participating funeral home can access those account records.
Your questionnaire link acts as a private access link. Anyone you share it with can submit details and, once released, download the finished song. Keep it within your family and funeral-home team. Public questionnaire forms without a partner link retain the email/share process.
